Asper our phone conversation today, I am enclosing information or statements with regards to the Conditions of Approval for completing the USR. Your letter of February 18, 1999, outlined the Conditions that still needed to be addressed. The Conditions are as follows: ~/ 3b. The Weld Coztnty• Health Department has approved a system of monitoring wells to detect impacts on nearby groundwater uses and residents. As we discussed, a monitoring plan has been submitted to Trevor. We are awaiting his approval. j 3c. An emissions permit has been obtained from the Air Pollutiotz Control Division, Colorado Department of Health, for fugitive dust and source emissions. The approved emissions permit has been obtained. A copy is attached. 3d. A NPDES Permit has been obtained for the Water Quality Control Division of the Colorado Department of Health. The approved NPDES has been obtained. A copy is attached. 3f. A 409 Permit has been issued by the .9rmy Corps of Engineers, if applicable. As we discussed, no regulated activities will disturb jurisdictional wetlands therefore, a 404 Permit is not necessary. Sa. Prior to recording the Special Review Plat, mt off-site Road Improvements and Maintenance Agreement shall be approved by the Board of County Commissioners. This agreement has been submitted to the Weld County Public Works Department. We are awaiting approval.
