06025 Banks and Gesso, LLC <br />Exhibit M -Other Permits and Licenses ~~~~~~ <br />Additional permits and approvals required for the Overlook Mine are listed below. ~CT 20 zoos <br />DivisiO~ °f <br />Local Permit ~~nmeaRdgaety°n, <br />The subject property has been annexed into the Town of Granby. Use of the property <br />for sand and gravel mining is permitted via an Annexation and Development Agreement <br />for the property, dated March 5, 2003, and the Planned Development Overlay District for <br />Granby Ranch. <br />CDPS Stormwater Discharge Permit <br />The applicant will apply for and obtain a Stormwater Discharge Permit from the Colorado <br />Department of Public Health and Environment's Water Quality Control Division. A <br />Stormwater Management Plan will be created and implemented for this site. <br />Air Permits <br />The applicant will submit an Air Pollution Emission Notice (APEN) for Fugitive Dust and <br />obtain air emission permits for the on-site processing equipment from the Colorado <br />Department of Public Health and Environment's Air Pollution Control Division. <br />Substitute Water Supply Plan/Well Permits <br />This operation will not use or encounter groundwater, therefore a gravel pit well permit or <br />Substitute Water Supply Plan is not necessary. <br />State Historic Preservation Office Clearance <br />Please see the results of a search of the Colorado Inventory of Cultural Resources in <br />Attachment A. There are no cultural resources located within the permit boundary. <br />Spill Prevention Control and Countermeasure (SPCC) Plan <br />The applicant will have a portable diesel fuel tank located on site of 1,000 gallons or less <br />so a SPCC Plan will not be required.
