6.4.6 EXIBIT F -Reclamation Plan Maa <br />13. The Exhibit F map provided on 5/14/03 was reduced in size to a 11" x 14" format. The <br />1"=200' scale is no longer accurate and the detail of the map is difficult to read. Please provide <br />this exhibit in the origina124" x 36" format with accurate map scale. <br />6.4.7 EXHIBIT G -Water Information <br />14. Please provide a written response to the objectors' comments regarding adverse affects to <br />groundwater (see attached comments). <br />15. The applicant states on page 22 that the operation is expected to drrectly affect surface water <br />or groundwater systems. Although the applicant specifies that installation of a slurry wall prior to <br />exposure of groundwater should alleviate any possible adverse impacts to groundwater in the <br />surrounding area due to dewatering, the applicant has not provided any analysis ofpossible <br />hydrologic impacts to surrounding property owners and well owners such as mounding and/or <br />shadowing. Further, the applicant has not provided an assessment ofpossible adverse hydrologic <br />impacts to surrounding property owners and well owners in the event of a slurry wall failure, ie: <br />leakage in excess of State Engineer's Office criteria. In order to resolve these concerns, the <br />Division requests that the applicant provide a revised analysis ofpossible hydrologic impacts, <br />and a groundwater monitoring plan that provides the Division reasonable assurance that any <br />possible mining related hydrologic impacts throughout the life of the operation will be detected <br />and mitigated. Please respond. <br />16. Reclamation Plan Map, Exhibit E shows the Plumb Ditch to remain intact during final <br />reclamation and will be within 30' of the slurry wall and 70' of the reclaimed shoreline, ht <br />conformance with Rule 6.4.7(2)(a), please indicate whether the adjacent Plumb Ditch is lined or <br />unlined and what the present usage of the ditch is. <br />17. In conformance with Rule 6.4.7(6), please identify and characterize the known aquifer at the <br />site. <br />18. The applicant has provided a location map of 16 adjacent wells and corresponding list of well <br />owners for groundwater wells within 600 feet of the site. Please verify whether any of these 16 <br />wells occur within 200 feet of the proposed affected land. <br />19. The applicant specifies on page 18 that HI has applied for a well permit and temporary <br />substitute supply plan. Please commit to no mining within two (2) feet of the highest recorded <br />groundwater table level until the operator (lam has proved to the satisfaction of the Division of <br />Minerals and Geology that they have been issued a valid well permit and temporary substitute <br />supply plan from the Office of the State Engineer. Attached is a copy of review comments from <br />the Office of the State Engineer. <br />
