Application
Dear Ms. Stout:
On November 5, 2001, the Division of Minerals and Geology received Application for Succession of Operators
form, a $100.00 fee, and a 112c permit apphcation including exhibits. The Division is returning the Application
for Succession of Operators for the following:
The Division requires that a Transfer of Approved Mineral Application form be submitted rather than
the form originally submitted.
The fee for a transfer is $125.00, therefore, the Division requires that an additional $25.00 be submitted.
The Division is also returning Page 1, 3, 5, 6 and the Signature page of the 112c permit application form for the
following:

The Division requires that the Applicant/Operator names remain consistent throughout all documents
(i.e. Diana K. Watson vs. Diana K. Stout).
