Rio Blanco County <br />5 Iaol <br />Enclosed please find a copy of the permit issued under the Federal Clean <br />Water Act and Colorado Water Quality Control Act. <br />Issuance of this permit constitutes a final determination by the Division <br />of Administration of the Colorado Department of Health, in conjunttion with <br />the U. S. Environmental Protection Agency and may be subject to administrative <br />review proceedings pursuant to the State Administrative Procedure Act, in- <br />cluding an adjudicatory hearing. You are advised to consult this act and <br />particularly to consult Sections 24-4-104, 24-4-105, 24-4-102(7), and 25-8-401, <br />G.R.S. 1973 for more information. In addition, the Regulations for the State <br />Discharge Permit System contains material that is pertinent to any admini- <br />strative review of the issuance of this permit. <br />Your NPDES Waste Discharge Permit required that specific action be performed <br />at designated times. Failure to meet these requirements constitutes a vio- <br />lation of this permit and can result in civil and/or criminal action(s). <br />Please read the permit very thoroughly. <br />All municipal and industrial facilities are required to submit self- <br />monitoring information. (PART I.B. Monitoring and Reporting). <br />Frequencies and types of self monitoring are summarized in PART I.A. <br />Effluent Limitations and Monitoring Requirements. <br />2.
