FOREST SERVICE EVALUATION OF PLAN OF OPERATIONS Continuance of FS-2800-5, Plan of Operations for Mining Activities on NFS lands A. Required Changes/modifications/special mitigation for plan of operations: In Reference to the proposed Plan of Operation for Crystal Jack submitted by Joseph Dorris on June 30, 2005. ' I. GENERAL INFORMATION E. Expected durafion of operation is £ve years from the signature date of the proposed Plan of Operation by the authorized officer. G. Expected date for completion of reclamation will be November 30.2011. IV. DESCRIPTION OF OPERATIONS A.'Access: FS Clarification/Revision: • New temporary road access should be in accordance with the Decision Nofice signed by Brent Botts on June 7, 2006. • The operator is required to request percussion from private property owners prior to utilizing a private access road, constructing any temporary access roads or mining on private property. Proof of written consent should be provided to the Forest Service and State and Local agencies prior to use. • The operator may request aRight-of--Way permit to access closed system roads in order to access his mining claim. • The operator must use necessary precautions to not damage any utility lines or associated facilities en route to dig sites in the project area. • Equipment maybe walked-in off system roads. If tracks or road development becomes evident an additional bond maybe required for continued operation. • No system road maintenance is authorized without approval of the Forest Service authorized officer. C. Project Description. FS Clarification/Revision: Timber • All trees over 8" in diameter must be designated for removal prior to excavation of dig sites and must receive Forest Service approval. • Fallen trees must be limbed, lopped and scattered. Slash will be no deeper than 12" on the ground. Bole material lazger than 4" in diameter will be removed to location where the public can access it for fuewood. In lieu of cutting, trees may be pushed or excavated with mechanized equipment. All resulting stumps must be buried in the adjacent dig site when reclamation occurs. • The operator would be required to purchase a firewood permit to use any cut trees that are not duectly incident to mining operations. Fire: • All intercal combustion engines including but not limited to generators, vehicles, mechanized mining equipment and power tools, must be fitted with an approved spazk arresting device or muffler in good working order. • The operator must have and maintain hand tools for fire suppression in the event of a wildland fue.
