PART I Page 25 Permit No. CO-0(11X1221 • E. GENERAL MONITORING, SAMPLING AND REPORTING REQL1QtEMENTS 6. Sienatorv and Certification Requirements a. All reports and other information required by the Division, shall be signed and certified for accuracy by the permittee in accord with the following criteria: i. In the t:ase of corporations, by a principal executive duly authorized representative, if such representative which the discharge described in the form originates; officer of at leazt the level of vice-president or his or her is responsible for the overall operation of the faciliq• from ii. In the case of a partnership, by a general partner; iii. In the case of a sole proprietorship, by the proprietor; iv. In the caze of a municipal, slate, or other public facility, by either a principal executive officer, ranking elected official, or other duly authorized employee. All reports required by permits, and other information requested by the Division shall be signed by a person az described above or by a duly authorized representative of that person. A person is a duly authorized representative only if: i. The authorization is made in writing by a person described above: ii. The authorization specifies either an individual or a position having responsibility for the overall operation of the regula[ed facility or activity such az the position of plant manager, operator of a well or a well field, • superintendent, position of equivalent responsibility, or an individual or position having overall responsibility for environmental matters for the company. (A duly authorized represenative may thus be either a named individual or any individual occupying a named position); and, iii. The written authorization is submitted to the Division.
