Routine Reoortine of Data <br />Reporting of the data gathered in compliance with Part I.B.1 shall be on a quarterly basis. Reporting of all data <br />gathered shall comply with the requirements of Pan I.E. (General Requirements). Monitoring results shall be <br />s„mma,;„Yi for each calendaz quarter and reported on Division approved dischazge monitoring report (DMR) forms <br />(EPA form 3320-1). The forms shall be mailed to the agencies listed below so they aze received no later than the 28th <br />day of the month following the end of the quarter (far example, the DMR for the first calendar quarter must be received <br />by the Division by April 28th). If no discharge occurs during the reporting period, "No Discharge" shall be reported. <br />The DMR forms consist of four pages -the top "original" copy, and three attached no-carbon-required copies. The Discharge Monitoring Report forms shall be filled out <br />accurately and completely in accordance with requirements of this permit and the instructions on the forms. They shall <br />be signed by an authorized person as identified in Pan I.E.6. <br />Calculations for all limitations which require the averaging of measurements shall utilize an arithmetic mean unless <br />otherwise speed by [he Division in the permit. <br />'. Representative Sampline <br />Samples and measurements taken as required herein shall be represen[ative of the volume and nature of the monitored <br />discharge. All samples shall be taken at the monitoring points specified in this permit and, unless otherwise specified, <br />before the effluent joins or is diluted by any other wastestream, body of water. or substance. Monitoring points shall <br />not be changed without notification to and approval by the Division. <br />If the permi[tee monitors at the point of discharge an}' pollutant limi[ed by the permit more frequently than required by <br />the permit, using approved tes[ procedures or as specified in the permi[, the result of this monitoring shall be included in <br />the calcula[ion and reporting of data to the Division. <br />J <br />INDBP s/9/93 <br />
