b. Summary of Application: The application requires, at a minimum, the following:

1) Name, and addrrae of the applicant;
2) Site address and lorstion;
3) Facility wntact person sad telephone number;
4) Standard Industrial Classification (SIC) code(s);
S) The sin of the mining operation in acrca;
6) Scotus u active or inactive;
7) list of any other mvironmeatal permits currently Geld by the facility;
S) Activities which talcs place at tho site;
9) Receiving waters; and
10) For new aetive mining operations (begimriag operations after 10/1/92) or existing facilities applying after
7/1/93, certification as to the preparation of a SWMP. For inactive mining operations applying after July
1, 1994, certifintion as to the preparation sad implementation of a SWMP.
c. Certification of SWMP Completion and Implmtentation
For active mining operations, existing on October 1, 1992, the permittce shall have implemented and be in
compliance with their SWMP, as soon as possible but no later than the eaniversary date of their Mined Land
Reclamation permit that falls between July I, 1993 and June 30, 1994. Certification to this effect must be
received by the Division on or before that date.
For new active faulitie5 (begiming operations after October 1, 1992), and for all other active facilities applying
after ]uly 1, 1993, the SWMP shall be prepared prior to applying for coverage tinder the general permit.
For inactive mining operatioas, applying before July 1, 1994, as soon as possible but no later than July 1, 1994,
the pemtittce shall have implemented and be in compliance with their SWMP. Certification to this effect must
6e received by the Division en or before that date. For those applying after July 1, 1994, the SWMP shall be
preparod prior W applying for coverage under the general permit.
4. Permit Certifimtion ProcedtQes
If this general permit is appropriate for the applicant's operation, then a certification will be developed and the applicant
will be authorised to discharge atarmwater under this general permit.

a. Request for Additional Information: The Division shall have up to thirty days after receipt of the above
information to request additioml data and/or dmy the authorisation for any particular discharge. Upou receipt
of additional information, the Division shall have an additional Fdteen days to issue or deny authorisation for the
particular discharge. Temporary coverage order this general permit will be allowed until the individual permit
goes into eflcet.
b. Atrtatnatic Coverage: If the appGcaat does not receive a request for additional information or a notification of
denial form the Division dated within thirty days of receipt of the application by the Division, authorisation to
discharge is accordance with the conditions of this permit shall be dcemed granted.
