BEEN ISSUED BY THE DIVISION AFTER RECEIPT OF YOUR PERFORMANCE WARRANTY. A
PERMIT WILL NOT BE ISSUED UNTIL WE VERIFY THE ADEQUACY OF THE PERFORMANCE
