-~- EXHIBIT F - Proof of Compliance with Local Government Zoning In accordance with 34-32-115(4)(e), the Board must malke a finding that the operation will be in compliance with local zoning or subdivi5;iian regulations. You a e not required to submit this information, but a submiss9'on would assist the Div sion's processing of the application. When the Division notifies the local gover ment of the existence of this application, it will request information regarding compl ance with applicable local law. It is intended that operators may concurrently came into compliance with local requirements at the same time the Division is proves ing this application. Information regarding the status with local government will a helpful. EXHIBIT G - Source of Legal Right-to-Enter ~s could be a copy of. the lease, deed, abstract of title, or current tax receipt. An ceptable alternative would be a statement acknowledged by a Notary Publi that the operator has the legal right to enter the mine. EXHIBIT H - Reclamation Costs Fora 110 permit, if the operator requests financial warranty to be less tan E5,000.00, the operator must submit facts to support such a request. Otherwise, a fi ancial warranty of E5,000.00 will automatically be set. For all 111 permits, the financial warranty shall be 52,500.00 per acre.
