In the interest of efficient processing, <br />please check the appropriate box to indicate which type of application y u are applying <br />for. 111 permits may be issued only under special circumstances. Be su a you are <br />eligible. <br />ICI LIMITED IMPACT OPERATIONS must disturb less than ten acres (in luding all <br />roads, stockpile areas and any other surface facilities) over he life of the <br />mine, and must extract LESS THAN 70,000 tons of mineral, overb rden or <br />combination thereof per calendar year. If the affected area i some fraction <br />greater than nine acres, be sure NOT to round this figure up t ten acres in <br />the narratives or elsewhere. <br />I_I SPECIAL (ten-day processing) operations are any sand, gravel, o quarry <br />aggregates operations which are to be operated for the sole pur ose of <br />• obtaining materials for highway, road, utility, or similar-type construction <br />under a Federal, State, county, city, town, or special district contract where <br />the contract calls for work to be commenced within a specifical y short time <br />and which will affect ten acres or less (including ail roads, s ockpile areas, <br />and any other surface facilities). <br />INSTRUCTION: This application and all accompanying materials as outlined below must be <br />• submitted in duplicate.
