6.3.1 E7~IT A - I.eaal 1)~crlotbn and Incatlon Mao
(1) The legal description must identify the affected land, specify
affected areas end be adequate to field locate the property. Description
shall be by township, range, and section, to at least the nearest quarter-
quartcr section. A metes and bounds survey description is also acceptable.
Also, the street add[ess or lot number(s) tray be used. This information is
available from the County Assessor's o1~ce or U. S, Geological Survey
(VSGS) maps.
(2) A map showing information sufficient to determine the
location of the affected land on the ground and existing and proposed roads
or access routes to be used in connection with the mining operation.
Names of all immediately adjacem suefacc owners of record shall also be
shown. The operation location map shall be a standard 1:24,000 scale U.S.
Geological Survey map. Tfie location of the proposed operation shall be
shown and labeled with the mitre site name.
6.3.2 E%H1BIT B -Site Ikecritstion
Items (a)-(c) below must be addressed to the extent necessary to
demonstrate compliance with the applicable performance standard
requirements of Rule 3. At a minimum, the Operator/Applicant shall
include the following information:
(a) a description of the vegetation and soil characteristics
in the erne of the proposed operation. The local office of the Soil
Conservation Service (SCS) may provide you with this information
as well as recommendations for Exhibit D - Rcehunation Plan;
(b) identify any permanent man-made structures within
two hundred (200) feu of the affected area and the owner of each
structure. Hach structure should be located on Exhibit E -Man:
(c) a description of the water resources in the sera of the
proposed operation. Identify any streams, springs, lakes, stock
water ponds, ditch, reservoirs, and aquifers which would receive
drainage directly from the affected area. Provide anY information

(d) A wildlife statement prepared by the Colorado
Division of Wildlife (DOW) is not required for Limited Impact
