Cattany <br />Division Director <br />Natural R~ urce Trustee <br />No. M-2004-048, Receipt of Incovm~plete 110 <br />On Iune 30, 2004, the Division of Minerals and Geology received your the 110 Constntction Materials Reclamation Permit <br />Application package for the Cedar Point Picture Rock Quarry, File No. M-2004-048. Preliminary review of the information <br />received determined that two (2) copies of the following items must be received before the Division can consider your application <br />as being submitted and technical review can begin: <br />APPt.t(`ATiON FORM: The application form received is no longer accepted as a valid form by We Division and the <br />Mined Land Reclamation Board. Anew form is enclosed for your use. This new form currently is accepted. The <br />Division requires that an Original Appliprion signed in blue ink and a copy of the original be submitted. <br />F.XAiRTT F. -Man Qtnle fi.3.5L• (1) In addition to the requirements of 6.2.1(2), you must provide a map that clearly <br />descflbes the features associated with the mining plan and the components of the Reclamation Plan. Include one map <br />for the mine plan and one map for the Reclamation Plan. The map(s) must be drawn to a scale no smaller than <br />appropriate to clearly show all elements that aze required to be delineated by the Act and these Rules; show a north <br />arrow, note ary section corners adjacent to the proposed operation, and indicate the date illustrated At a minimum, <br />maps must include the following information: <br />(3) Reclamatinn Plan Man <br />(a) show the gradient of all reclaimed slopes (horizontal:vertical) sufficient to describe the post mine <br />topography; <br />(b) indicate where vegetation will not ire established and t11e general area(s) for shrub or tree planting; <br />(c) if ponds are a part of the Reclamation Plan, outline the final shore configuration of the ponds and <br />shallow areas if the future land use is for wildlife; <br />(d) slate the average thickness of replaced overburden by reclamation area or phase; and <br />(e) state the average thickness of replaced topsoil by reclamation area or phase. <br />F.XtitRTT C. - CnnLy;e of irgal Rig _ ,>_F.n er nlr 6_~ ~ provide a description of the basis for legal right of entry <br />to the site and to conduct mining and reclamation, for the Owners of Record described in Subparagraph 1.6.2(1)(e)(i). <br />This may be a copy of access lease, deed, abstract of title, or a current tax receipt. A signed statement by the Landowner <br />and acknowledged by a Notary Public stating that the Operator/Applicant has legal right to enter and mine is also <br />acceptable.
