Long <br /> <br />Re: Sha.ngri La Pit Permit Application, File No. M-89-024 IJinsion Director <br />Dear Mr Yaklich: <br />As we discussed during our July 28, 1998 phone conversation, you submitted an application fora 110 <br />Limited Impact permit in February, 1989. The application was approved in April, 1989 subject to a <br />$5,000.00 bond. However, since you never submitted a financial warranty ($5,000.00) or a performance <br />warranty, the permit never issued. <br />The Division inspected the proposed mine site April 9, 1998. A copy of the inspection report was mailed <br />to you. You confirmed receiving the report. Since you did not respond to my requests (noted in the <br />repott), the Division decided to place the item on the July 29-30, 1998 Mined Land Reclamation Board <br />agenda I:o have the Board reconcider their previous approval of the application, pursuant to the <br />requirements of Rule 4.1(2). The Division planned to ask the Board to terminate the file by denying the <br />application. <br />However, you now state that you do not want the file tetmininated. You would rather have a permit <br />issued [Iran to go through the application process again. As we discussed, all you need to do is submit a <br />properly executed financial warranty, in the amount of $5,000.00, and a performance warranty and a <br />permit will be issued. I have enclosed a Corporate Surety financial warranty form and a performance <br />warranty form for your use. You need to complete the forms and submit them to the Division within 60 <br />days (by September 26, 1998). <br />You mentioned that your wife passed away. Since she was one of the applicants, you will need to <br />complete a Request for Transfer of Approved Mineral Application and Succession of Permit Applicant <br />form (elclosed) to transfer [he approved application to you. You will need to submit a copy of the "death <br />certificate" for your wife in order to complete the transfer process.
