The access shall be constructed, maintained and used in <br />accordance with this permit, including the Stale Highway Access Code and any attachments, terms, conditions and exhibits. This permit may be revoked <br />by the issuing authority if at any time the permitted access and Its use violate arty parts of this permit. The issuing authority, the Department antl their duly <br />appointed agents and employees shall be held harmless against any action for personal injury or property damage sustained by reason of the exercise of <br />the ermit. <br />Location: Access is to be located on the East side of State Highway 285, a distance of approximately 2635 feet North of <br />Milepost 101. <br />Access to Provide Service to: To serve a maximum of one (1) gravel pit operation on the SE1/4 SE1/4 Section 26, <br />Township 46 North of Range 9 East of the New Mexico Principal Meridian, Saguache County, Colorado. <br />Other terms and conditions: <br />MUNICIPALITY OR COUNTY APPROVAL <br />Required only when the appropriate local authority retains issuing authority. <br />6y Date Title <br />(X) <br />Upon the signing of this permit the permittee agrees to the terms and conditions and referenced attachments contained <br />herein. All construction shall be completed in an expeditious and safe manner and shall be finished within 45 days from <br />Initiation.
