Dear Mr. Goode:
Cln April 6, 2001, the Division of Minerals and Geology received additional materials to your 110 Construction
Materials Reclamation Permit Application package for the B ~& D Tmcking site, File No. M-2000-072. Preliminary
review of the information received determined that the following items must be received before the Division can
consider your application as being submitted and technical review can begin:
APPLICATION FORM: Page 1, Item 1: Needs completing. Page 1, Item 2: Needs to be the same as the
name on the original application form. Page 6, Signature Page: The Division tequira that the name of the
applicant be consistent throughout this document (i.e. either Dorothy and Robert Goode or B & D Tmcldng).
EXHIBIT I -Proof of Filine with County Clerk (Rule 6.3.9): Provide an affidavit or receipt indicating the
date on which the new application was placed with the local County Clerk and Recorder. Also, refer to
Rule 1.6.2(1)(c).
Your application will not be considered filed until the information listed above is received and found sufficient to begin
our review. A decision date will be established 30 days from the date of receipt of all of the requested information.
Additionally, if you have already published notice you will need to republish notice, after the Division considers the
application submitted. Republication must occur once within ten (10) days of the submittal date. We will notify you
when you should initiate republication of your notice. The final date for receiving comments is ten (10) days after the
date of publication or the next regular business day.
This letter shall not be construed to mean that there are no other technical deficiencies in your application. The
Division will review your appliption to determine whether it is adequate to meet the requirements of the Act after
submittal of all required items.
