Long <br /> Division Director <br />Re: B & D Trucking, File No. M-2000-072, Receipt of 1]0 Construction Materials Reclamation Permit <br />Appticatioo Package <br />Dear Mr. Goode: <br />Cln April 6, 2001, the Division of Minerals and Geology received additional materials to your 110 Construction <br />Materials Reclamation Permit Application package for the B ~& D Tmcking site, File No. M-2000-072. Preliminary <br />review of the information received determined that the following items must be received before the Division can <br />consider your application as being submitted and technical review can begin: <br />APPLICATION FORM: Page 1, Item 1: Needs completing. Page 1, Item 2: Needs to be the same as the <br />name on the original application form. Page 6, Signature Page: The Division tequira that the name of the <br />applicant be consistent throughout this document (i.e. either Dorothy and Robert Goode or B & D Tmcldng). <br />EXHIBIT I -Proof of Filine with County Clerk (Rule 6.3.9): Provide an affidavit or receipt indicating the <br />date on which the new application was placed with the local County Clerk and Recorder. Also, refer to <br />Rule 1.6.2(1)(c). <br />Your application will not be considered filed until the information listed above is received and found sufficient to begin <br />our review. A decision date will be established 30 days from the date of receipt of all of the requested information. <br />Additionally, if you have already published notice you will need to republish notice, after the Division considers the <br />application submitted. Republication must occur once within ten (10) days of the submittal date. We will notify you <br />when you should initiate republication of your notice. The final date for receiving comments is ten (10) days after the <br />date of publication or the next regular business day. <br />This letter shall not be construed to mean that there are no other technical deficiencies in your application.
