f~
STATE OF COLORAD
DEPARTMENT OF TRANSPORTATION
Region I -Traffic Section
18100 East Colfax Ave.
Aurora, Colorado 8001 I
(303) 757-9126
FAX: (303) 343-0596
January 21, 2005
Mr. Phillip Wolf
P.O. Box 16804
Golden, CO 80402
Re: Request for extension of Access Permit 103039
Dear Mr. Wolf,
'w ~~,
Thank you for your letter requesting an extension of your Access Permit No. 103034. Regrettably, your Access
Permit expired on May 29, 2004. In order for CDOT, or a local issuing authority, to consider an extension of your
Permit, we need to receive your request in writing prior to the date of expiration. Your request was received on
January 12, 2005. Please reference Sectiori 2.3(11)(d) of the State Highway Access Code available on the web at
www.dot.state.co.us/AccessPer[nits/index.htm) , or Page 2 o_f your State Highway Access Pe_nnit Number 103039,
under "Peinilt Expiration". -
During our phone conversation on January 2t, 2005, you referenced a letter I had written you dated December 15,
2003. The letter was written as official notification that we agreed to remove Number 7 in the original "Terms and
Conditions" of your Permit, even though you signed your Access Permit on May 28, 2003, with that Term and
_ Condition included. My letter did not imply any extension of.the original.expiration date. However, if it had, the -
Permit would have expired one year after the date of the letter, or December 15, 2004. For this reason, the status of
your Access Permit as expired does not change in either case.
In order to reestablish an access permit that has expired, you will need to begin again with the application
procedures. However, Gilpin County is now the "Issuing Authorit}~' for all State Highway Access Permits within
their jurisdiction in cooperation with CDOT, as per C.R.S: Section 43-2-135 and 43-2-147 (see attached). I have
included a new State Highway Access Permit Application form with instructions for your convenience.
I apologize for any inconvenience this may cause you. You can reach me at 303-757-9126 with any questions.
mcerely, '' '' 11
'/vtn~cti W~
Anna Welch
Region One Access Manager
CC: Tony Petersen, Gilpin County
Region
File
RF
HQ
Attachments
= EXHIBIT
d
