Based on my <br />inquiry of the person or parsons who manage the system, or those <br />persona directly responsible for gathering, the information submitted <br />is, to the beet of my knowledge and belief, true, accurate, and <br />complete. i am aware that there are significant penalties for <br />submitting false information, lncludinq the possibility of fine and <br />imprisonment for knowing violations." <br />• PART II <br />A. HANAGEHENT REQUTAEIiENTS <br />1. Change in Discharge <br />The permittee shall inform the Division (Permits and Enforcement <br />Section) in writing of any intent to construct, install, or alter any <br />process, facility, or activity that is likely to result in a new or <br />altered discharge, and shall furnish the Division such plane and <br />specifications which the Division deems reasonably necessary to evaluate <br />the effect on the discharge and receiving stream. <br />The permittee shall submit this notice within two (2) weeks after making <br />a determination to perform the type of activity referred to in the <br />preceding paragraph. Process modifications include, but are not limited <br />to, the introduction of any new pollutant not previously identified in <br />the permit, or any other modifications which may result in a discharge <br />of a quantity or quality different from that which was evaluated in the <br />drafting of the permit including subsequent amendments. Following such <br />notice, the permittee shall be required to submit a new CDPS application <br />and this application may be processed ae an application for an <br />individual permit if the application shows that the new discharge ie <br />ineligible for coverage under the general permit. in no case shall the <br />permittee implement such change without first notifying the Division. <br />i <br />Amended in/li/44 Effective iTriaig~ <br />
