Item 28 -Potential contamination plume in groundwater. -The operator intends on minimizing
any impacts through the development of a Spill Prevention, Countermeasures and Control Plan
for the site per Item 31.
Item 29 - "Worst case" leak. -The operator intends on minimizing any impacts through the
development of a Spill Prevention, Countermeasures and Control Plan for the site per Item 31.
Item 30 -Monitoring plan to detect leaks -The operator intends on providing monitoring for
leaks through the development of a Spill Prevention, Countermeasures and Control Plan for the
site per Item 31.
]tem 31 -Spill containment and countermeasures plan. The operator will develop a Spill
Prevention, Countermeasures and Control Plan prior to beginning mining on the site. The plan
will contain the notification procedures, clean up procedures for a large leak and housekeeping
measures for in the event of minor leakage or spills.
Item 32 - StorTnwater Management Plan - If one is needed beyond the National Pollutant
Discharge Elimination System (NPDES) permit, then the operator will secure the Stormwater
Discharge Permit prior to mining.
Item 33 - NPDES permit. -The operator will obtain a National Pollutant Discharge Elinnation
System (NPDES) permit from the Water Quality Control Division, Colorado Departtnent of
Health prior to discharging water from the Mamm Creek Sand & Gravel Pits.
Rule G.4.10 Exhibit J -Vegetation Information
Item 34 -Location of vegetation types. This information has been supplied on Exhibits C-1 and
C-2. The location of the vegetation types is based on the aerial photography taken in 1999, on
site observations and wetlands delineation study previously submitted to the Army Corps of
Engineers.
Rule 6.4.13 Exhibit M -Other Permits & Licenses
Item 35 -Other permit status. The following is an update to the status of the other permits
information supplied in the permit application.
1. CDOT Access Permits -The Grand Junction office was contacted by phone on
December 22, 2000. CDOT personnel indicated that the terms and conditions
for tl~e perniits were being prepared. Based on a phone conversation on
December 26, 2000, it is our understanding that the access permits are ready
for final review and approval.
2. Army Corps of Engineers 404 permit -This permit application is in its final
draft form and will be subntted in January, 2001.
3. Garfield County Special Use Permit -This permit application was submitted on
September 7, 2000 and is currently in the review and approval process.
