The operator will develop a Spill <br />Prevention, Countermeasures and Control Plan prior to beginning mining on the site. The plan <br />will contain the notification procedures, clean up procedures for a large leak and housekeeping <br />measures for in the event of minor leakage or spills. <br />Item 32 - StorTnwater Management Plan - If one is needed beyond the National Pollutant <br />Discharge Elimination System (NPDES) permit, then the operator will secure the Stormwater <br />Discharge Permit prior to mining. <br />Item 33 - NPDES permit. -The operator will obtain a National Pollutant Discharge Elinnation <br />System (NPDES) permit from the Water Quality Control Division, Colorado Departtnent of <br />Health prior to discharging water from the Mamm Creek Sand & Gravel Pits. <br />Rule G.4.10 Exhibit J -Vegetation Information <br />Item 34 -Location of vegetation types. This information has been supplied on Exhibits C-1 and <br />C-2. The location of the vegetation types is based on the aerial photography taken in 1999, on <br />site observations and wetlands delineation study previously submitted to the Army Corps of <br />Engineers. <br />Rule 6.4.13 Exhibit M -Other Permits & Licenses <br />Item 35 -Other permit status. The following is an update to the status of the other permits <br />information supplied in the permit application. <br />1. CDOT Access Permits -The Grand Junction office was contacted by phone on <br />December 22, 2000. CDOT personnel indicated that the terms and conditions <br />for tl~e perniits were being prepared. Based on a phone conversation on <br />December 26, 2000, it is our understanding that the access permits are ready <br />for final review and approval. <br />2. Army Corps of Engineers 404 permit -This permit application is in its final <br />draft form and will be subntted in January, 2001. <br />3. Garfield County Special Use Permit -This permit application was submitted on <br />September 7, 2000 and is currently in the review and approval process. <br />
