The plan <br />was received on November 17, 1998 and was assigned serial number COC-62522. <br />Please refer to this number in future correspondence. <br />We completed the environmental assessment (EA) for the 3809 Plan of Operations for <br />the Van 4 Mine project area . The Plan of Operations includes previous mining <br />activities on the Van, Rodman, Paris and Curley mining claims. The area was <br />previously permitted under 34 CFR 3809 as a plan of operations to Umetco <br />Corporation. Anew serial number has been assigned to International Uranium <br />Corporation for this project. <br />The decision is to approve the plan of operations as described in the CDMG Permit M- <br />97-032, as submitted, subject to the following mitigation measures. The mitigation <br />measures are required to ensure no significant impacts will occur. <br />• The claimant/operatortyill notify the 8LM at least 15 days prior to any surface <br />disturbance of the proposed ventilation site so the BLM can conduct a cultural <br />resource survey of the proposed ventilation site when the location of the site is <br />known. <br />• The claimanUoperatortyill file an amendment to this plan of operations with the <br />BLM prior to any new surface disturbance outside of the permit area as required <br />by 43 CFR 3809. BLM will be notified of any changes to the mine permit <br />through plan amendments. <br />• The mine will operate and be reclaimed in accordance with the CDMG permit for <br />the Van 4 Shaft, M-97-032. <br />• Cultural resource and Threatened and Endangered Species inventories by BLM <br />are required prior to any surface disturbance outside of existing disturbance. <br />
