Long <br />Dwlslon Direnar <br />Re: Van 4 Shalt, File No. M-97-032, Receipt of 110(2) Bard Rock/Metal Mining Reclamation Permit <br />Application Paekage <br />Dear Ms. Rehmann: <br />On May 7, 1997, the Division of Minerals and Geology received your the I10(2) Hard Rock/Metal Mining <br />Reclamation Permit Application package for the Van 4 Shaft, File No. M-97-032. Preliminary review of the <br />information received determined that the following items must be received before the Division can consider <br />your application as being submitted and technical review can begin: <br />APPLICATION FORM: The application form submitted to the Division has not been used since <br />January, 1996. Anew Hard Rock/Metal Mining application form is enclosed for you to complete and <br />rctum to the Division, along with one copy. <br />EXHIBIT' G -Source of Letial Rieht-to-Enter (Rule 6.3.71: Provide a description of the basis for <br />legal tight of entry to the site and to wnduct mining and reclamation. This may be a copy of access <br />lease, deed, abstract of title, or a current tax receipt. A statement by the Landowner and <br />acknowledged by a Notary Public stating that you have the legal right to enter and mine is also <br />acceptable. The information submitted. for this exhibit is insufficient to meet the requiremenu of this <br />exhibit. . <br />EXHIBIT L -Permanent Man-Made Structures (Rule 6.3.121: You must provide information <br />sufficient to demonstrate that the stability of any structures located within two hundred (200) feet of <br />the operation or affected land will not be adversely affected. If the Office determines that such <br />information is inadequate to demonstrate that the operation will not adversely affect the stability of <br />any significant, valuable or permanent man-made structure, the Applicant shall either: <br />(a). provide a notariud agreement between the applicant and the person(s) having an <br />interest igthe structure, that the applicant is to provide compensation for any damage to the structure; <br />or <br />(b) where such an agreement can not be reached, the applicant shall provide an <br />appropriate engineering evaluation that demonsVates that such structure shall not be damaged by <br />activities occurring at the mining operation. <br />
