On May 7, 1998, the Division received an incomplete apphcation package for the Van 4 Shaft
Operation. On May 9, 1998, the Division received a copy of a certified mail return receipt card
which, according to the cover letter, was for the proof of providing notice of the application to
the San Miguel Basin Soil Conservation District. However, the copy of the return receipt cazd
received by the Division showed the wrong side of the return receipt card, and it was not possible
to verify that the addressee on the card was the Soil Conservation District. On May 20, 1997, the
Division sent International Uranium Corp. (then Energy Fuels Nucleaz, Inc.) a letter (copy
attached) listing application exhibits that were missing and explaining that those items must be
received before the Division could consider the application as being submitted and technical
review of the application could begin. In addition, the letter indicated that the wrong type of
application form had been used and as such the applicant needed to prepare a new apphcation
using the appropriate type of application form, a copy of which was attached to the letter sent to
the applicant by the Division. To date- the Division has not received anv of the nussina
apolication materials. nor has the Division received an avoronriately comoleted application form.
Also, since that time the name of the company has changed from Energy Fuels Nuclear, Inc. to
International Uranium Corporation and the applicant and permitting contact addresses and phone
numbers shown on the original application appear to have changed as well.
In order to continue the application process, the applicant needs to provide all of the missing
information listed in the Division's May 20, 1997 letter, correct the application form to show the
current company name, contact persons, addresses and telephone numbers, properly initial the
appropriate portions of the application form, complete the signaturelnotatuation portions of the
application form, and provide a copy of the correct side of the certified mail return receipt card
from the San Miguel Basin Soil Conservation District. The Division reiterates that this
information must be received before the Division can consider vour application as being_submitted
