MFICE CpPY SITE. h ~3z Pe'~: ~` Re: Van 4 Shaft, File No. M-97-032 Dear Mr. Oswald: Because we have had three different offices assisting with the preparation and submittal of [he Van 4 Permit Application over the last several years, I have attempted to collect information and documents from each office so that the Denver office of International Ltranium has the master set, and [o help verity that the state and county receive everything needed. Enclosed are documents to be included in the Van 4 Permit Application. Some may duplicate what has already been filed, but [want to verify that all requirements leave been met. The enclosed documents are listed as follows: I . Copies of the letters dated April 30, 1997, and the original, signed, green return receipt cards, of the mailings to the San Miguel Basin Soil Conservation District, the Board of County Conmrissioners and [he Clerk and Recorder's Office giving notice of, and a copy of, the Permit Application. 2. A copy of the Public Notice and the original, signed, green return receipt card showing the mailing of such notice to the Bureau of Land Management. This mailing did not include a cover letter. 3. The original Affidavit of Publication showing the publication of the Public Notice in the Montrose Daily Press. 4. A copy of the letter dated November ]0, 1998, to the Montrose County Clerk and Recorder submitting the revised Reclamation Permit, together with the original, signed, Receipt.
