Long <br />Dwision Dvecmr <br />On Apri15, 1999, the Division of Minerals and Geology received one original application form and one copy of your <br />the 110(2) Hard Rock/Metal Mining Reclamation Permit Application package for the White Bank Mine, File No. <br />M-99-042. Preliminary review of the information received determined that the following items must be received before <br />the Division can consider your application as being submitted and technical review can begin: <br />EXHIBIT E -Map (Rule 6351: (l) In addition to the requirements of 6.2.1(2), you must provide a map that <br />clearly describes the features associated with the components of the Reclamation Plan. The map must be drawn <br />to a scale no smaller than appropriate to clearly show all elements that are required to be delineated by the Act <br />and these Rules; show a north arrow, note any section comers adjacent to the proposed operation, and indicate <br />the date illustrated. At a minimum, maps must include the following information: <br />(3) Reclamation Plan Mao <br />(a) show the gradient of all reclaimed slopes (horizontal:vertical) sufficient to describe the post <br />minetopograph~; <br />(b) indicate where vegetation will S be established and the general area(s) for shrub or tree <br />planting; <br />(c) if ponds are a part of the Reclamation Plan, outline the fmal shore configuration of the ponds <br />and shallow areas if the future land use is for wildlife; <br />(d) state the average thicla[ess of replaced overburden by reclamation area or phase; and <br />(e) state the average thickness of replaced topsoil b}• reclamation area or phase. <br />EXHIBIT G -Source of Legal Right-to-Enter (Rule 6.3.71: Provide a description of the basis for legal right <br />of wtn to the site and to conduct mining and reclamation. This mav_ be a cope of access lease, deed, abstract <br />of title, or a current tax receipt. A statement by [he Landowner and acknowledged by a Notary Public stating <br />that you have the legal right [o enter and mine is also acceptable. This information .vas not enclosed in the <br />application package received. <br />EXHIBIT I -Proof of Filing with County Clerk (Rule 6.3.91: Provide an affidavit or receipt indicating the <br />date on which the application vas placed with the local Counh Clerk and Recorder. Also, refer to Rule <br />L6.2(I)(c). <br />
