Please check one below and remm this <br />form with yom payment to DMG. If a CD is requested and the permit is too large for a CD, you will be called. <br />Plea/se allow up to 5 business days from the receipt of this signed document for your CD or DVD to be available. <br />V CD @ $5.00 per disk _ DVD @ $20.00 per disk <br />Please check if you want a copy of the QuickView installation instructions. <br />Computer regpiremmb to use the DMG docameat imaging CD (or DVD): <br />1. A program called QuickView will 6e copied to the CD and mast be installed on your computer in order m use a DMG imaging CD the <br />first time. Future use of this or other imaging CD's does not require a new installation of the software. <br />2. The CD or DVD that you purchase ~ be in the D: drive. A DVD drive will read a CD or DVD. <br />3. The CD will contain files with the .tif extension. There needs to be ao image viewing application for .tif files on yom computer that <br />will handle single and multi-page documents. On a Windows XP computer, the default viewing programs are called "Windows Picture <br />and Fax Viewer" or "Paiat^ and they do not work with QuicltView. The Imaging Viewer that twines with pus-Windows XP computers <br />does work. DMG has determined that a program called "irfanView^, which can be downloaded from IrCanView.com, does work with <br />QuickV iew. After downloading "IrfanView^, it needs to be configmsd as yom defauh .tif viewer. DMG does not support or endorse this <br />software and the licensing and registration are the user's responsibility. There are many other programs that work as well, such as <br />"Imaging Preview^ and "Imaging for Windows^. <br />lastractions• <br />1. Insert the CD into yom computer. <br />2. Allow time for the CD to install. <br />3. Eject the CD from the computer. <br />4. Reinsert the CD into dte computer. <br />5. You are rsady to view the Permit Files. <br />NOTE: A program called QuickView Autorun, which raps the QrdckView <br />setup program, has also been copied ro this CD. Ifthe Autontn program <br />does NOT work, you will need to manually install the QuickView program. <br />Instructions for manually installing QuickView are available upon request <br />with the CD or can be viewed at www.minipa.state.co.us. <br />The Permit File dornmept images can be viewed in the reception area of the DMG office or they can be purchased op a CD or <br />bVD. The CD/DVD purchase cost covers the time and labor of making the CD.
