45. The coat for concrete disposal for the warehouse/shop/office foundation <br />was not included in the calculations. Please address. <br />46. The revegetation coats are calculated out at $737.50 an acre, however, no <br />records or calculations exist to justify this figure. This needs to be <br />included in the bond calculation. <br />Miscellaneous <br />47. Please provide an index identifying the location of all of the figures, <br />tables and maps within the entire permit document, to improve the <br />accessibility of information. Currently, figures, tables and maps found <br />in the Exhibits are not indexed anywhere in the permit. <br />48. The Division has had some difficulty maintaining the two office copies of <br />the permit application complete with regard to Exhibit 15, the Revision <br />Exhibit. Each revision submittal has a cover letter, revised text pages <br />in the permit application, a list of those revised pages, and occasionally <br />a separate Exhibit 15 narrative. The Division usually receives three <br />copies of everything. This usually means that one set of the permit <br />application goes without revised text pages in Exhibit 15. Please <br />consider the format that EFCI would find moat useful for Exhibit 15, and <br />send ue two sets dated back to the permit renewal submittal in early 1989, <br />so our permits could be consistent. <br />49. Several major modifications to the operations and reclamation plan have <br />been submitted and then either withdrawn or not approved. Consequently, a <br />number of permit document pages exist in the Division's copy of the permit <br />application that are now obsolete. Please remove all references to the <br />placement of less than 4' of cover on the refuse pile and to the placement <br />of refuse in the GEC East Pit. At a minimum, remove mention of either of <br />these topics from pages 2.05.4-iSRa, 2.05.3-41R, and Exhibit 9. <br />II. Status of Stioulations <br />STIPULATION NO. 1 <br />THE OPERATOR SHALL SUBMIT RIGHT OF ENTRY INFORMATION FOR THE FLORENCE CITY, <br />A.T. MASSEY, AND CORLEY PROPERTIES, WHERE SUBSIDENCE IS PROPOSED, 30 DAYS <br />PRIOR TO AFFECTING THESE LANDS. <br />Statue - Terminated <br />This stipulation was terminated with the issuance of the decision for Permit <br />Revision 1, March 11, 1991. Stipulation 12 was issued at that time to cover <br />additional required right of entry information. <br />STIPULATION NO. 2 <br />THE OPERATOR SHALL CONTINUE ALL HYDROLOGIC MONITORING OUTLINED ZN THE <br />APPLICATION AND THIS FINDINGS DOCUMENT UNTIL SUCH TIME AS THE DIVISION <br />APPROVES OF MODIFICATION OR CANCELLATION OF THE PROGRAM. <br />Statue - Terminated <br />The revised permit application, submitted in conjunction with the permit <br />renewal and approved 11/27/89, included a hydrologic monitoring plan as <br />Exhibit 25. <br />
