Please provide an index identifying the location of all of the figures, tables and maps within the entire permit document, to improve the accessibility of information. Currently, figures, tables and maps found in the Exhibits are not indexed anywhere in the permit. 48. The Division has had some difficulty maintaining the two office copies of the permit application complete with regard to Exhibit 15, the Revision Exhibit. Each revision submittal has a cover letter, revised text pages in the permit application, a list of those revised pages, and occasionally a separate Exhibit 15 narrative. The Division usually receives three copies of everything. This usually means that one set of the permit application goes without revised text pages in Exhibit 15. Please consider the format that EFCI would find moat useful for Exhibit 15, and send ue two sets dated back to the permit renewal submittal in early 1989, so our permits could be consistent. 49. Several major modifications to the operations and reclamation plan have been submitted and then either withdrawn or not approved. Consequently, a number of permit document pages exist in the Division's copy of the permit application that are now obsolete. MASSEY, AND CORLEY PROPERTIES, WHERE SUBSIDENCE IS PROPOSED, 30 DAYS PRIOR TO AFFECTING THESE LANDS. Statue - Terminated This stipulation was terminated with the issuance of the decision for Permit Revision 1, March 11, 1991. Stipulation 12 was issued at that time to cover additional required right of entry information. STIPULATION NO. 2 THE OPERATOR SHALL CONTINUE ALL HYDROLOGIC MONITORING OUTLINED ZN THE APPLICATION AND THIS FINDINGS DOCUMENT UNTIL SUCH TIME AS THE DIVISION APPROVES OF MODIFICATION OR CANCELLATION OF THE PROGRAM.
