"~ <br />CERTIFICATION OF PERMANENT CLOSURE <br />FRUITA #1 MINE PORTALS <br />The following certified statement describes the methods used to permanently <br />close the three entries of the Fruita #1 Mine. The portal closure was <br />completed by Dorchester Coal Company personnel during July, 1983. <br />Two seals were installed in each of the three entries. Kennedy steel stoppings <br />were erected about 80 ft. inby the portal in the intake and belt entry, and <br />about 50 ft. inby the return entry. (See Figure 1). These Kennedy stoppings <br />were sealed with a rigid urethane foam coating. Concrete seals were then <br />constructed 20 to 30 ft. outby of each of the Kennedy stoppings. (See <br />Figure 1). These permanent seals were constructed of two courses of solid <br />8"x16" concrete block. Block was laid on a concrete 3'x8" footing, and wet <br />grouted on all but the outby side. Footings and block were keyed into the <br />ribs of the entries. <br />Construction was identical to that in the enclosed drawing, (Figure 2), except <br />that dual rather than single pilaster construction was used in each seal. <br />Seals were constructed under the supervision of a brick mason. <br />The outby surface of the seals were coated with approximately 4 in. of <br />rigid urethane foam sealant. <br />I, hereby, certify that the above statement accurately describes the permanent <br />closure of the entries to the Fruita #1 Mine. <br />Name U~.'W. ~~;,,N,~,~ Date~~ /_~~ /9d'j~' <br />Ti tl e,~J,~,,..,,.._ ~;,, ,,, <br />The above statement was acknowledged before me this /~"'=~ day of <br />_ / 9B1~ by J.
