Petitioner's residence address, office address, and <br />office telephone number, the courts to which admitted, and the <br />respective dates of admission are provided, on the attached form <br />provided by the Clerk of the Court, as required. <br />5. Petitioner has not appeared as counsel in this Court in <br />any other case in the past five years. <br />6. The Case Management Order further provides that all <br />motions for admission pro hac vice are granted unless prompt <br />objections are filed.
