<br />NOV #C-99-010 <br /> <br />This violation was issued on October 28, 1999 for failure to conduct hydrologic <br />monitoring in accordance with the approved plan. This violation was terminated on <br />October 28, 1999. Penalty was set at $1,000.00 and was paid by Energy Fuels Mining <br />Co. on December 28, 1999. Therefore, no further action is required for this NOV. <br />NOV #C-99-011 <br />This violation was issued on October 29, 1999 for failure to conduct reclamation <br />operations in accordance with the approved plan. This violation was terminated on <br />October 18, 1999. Penalty was set at $750.00 and was paid by Energy Fuels Mining <br />Co. on December 28, 1999. Therefore, no further action is required for this NOV. <br />NOV #C-99-014 <br />This violation was issued on November 24, 1999 for implementing a revision prior to <br />final Division approval. This abatement date was extended with Director approval to <br />March 24, 2000 and following approval of Minor Revision No. 17 on March 24, 2000, <br />the violation was vacated by the Director. <br />Section V <br />Summary and Discussion <br />This section consists of questions or comments raised as a result of the Division's <br />review of the permit application and related materials. As allowed by Rule 2.08.3(3) the <br />Division will require the operator submit a technical revision responding to the issues <br />identified below. <br />Rule 2.03 Legal, Financial, Compliance and Related Information <br />1. Rule 2.03.6(1) requires a complete and detailed legal description of the permit <br />boundary. Please incorporate this information into the permit text and submit the <br />revised pages. <br />Rule 2.05 Operations and Reclamation Plans <br />2. Question #5 of the 1998 Annual Hydrology Report review letter (August 12, 1999) <br />requests a minor revision to address a number of concerns regarding hydrologic <br />monitoring.
