PART I. AUTHORIZATION TO CONSTRUCT AND OPERATE <br />Pursuant to the Underground Injection Control Regulations of the U. S. Environmental <br />Protection Agency codified at Title 40 of the Code of Federal Regulations, Parts 124, 144, 146, and <br />147, <br />Amenicm~ Soda, L.L.P. <br />2717 County Road 215 <br />Parachute, Colorado 81635 <br />is hereby authorized to operate aClass Inon-hazazdous disposal well, commonly known as the: <br />GetryNo..l-7LWDisposal Wel! <br />NE 1 /4 of the NE 1 /4 <br />1282 feet from the north line and 386 feet from the east line <br />Section 7, Township 8 South, Range 97 West, 6"' PM <br />Garfield County, Colorado <br />Injection shall be for the purpose of disposing of water from a Nahcolite solution mine processing <br />plant, consisting of partially saturated to saturated brine solution, including sodium carbonate <br />solutions, in accordance with conditions set forth herein. No hazardous waste material or waste <br />which is generated off site maybe injected into the well. Injection between the outermost <br />casing protecting underground sources of drinking water (USDWs) and the wellbore is <br />prohibited. If the well is not converted from its existing Class II status within one (1) year from <br />the effective date of this Permit, the Permit shall be terminated unless extended according to Part <br />II, Section A. 6. <br />Injection activities shall not commence until the operator has fulfilled all applicable <br />conditions of this permit and has received written authorization from the Director. "Prior to <br />Commencing Injection" requirements are set forth in Part lI, Section C. 1. of this Permit. <br />All conditions set forth herein refer to Title 40 Parts 124, 144, 146, and 147 of the Code of <br />Federal Regulations and are regulations that rare in effect on the date that this permit becomes <br />effective. <br />This permit consists of a total of 77 pages and includes all items listed in the Table of <br />Contents. Further, it is based upon representations made by the permittee and on other information <br />contained in the administrative record. Any information submitted by the permittee found to be <br />inconect may be cause for modification or termination of the permit and/or may subject the <br />permittee to formal enforcement action. It is the responsibility of the oermittee to read. <br />understand. and carry out all provisions of this Permit. <br />This permit and the authorization to inject are issued for a period often (10) years, unless <br />terminated (Part III, Section B); no reapplication will be necessary during this period, unless the <br />EPA Final Permit No. CO 10932-04664 <br />Page 5 of 77 <br />
