The insurance certificate covers the time period from January 2, 2004 <br />through January 1, 2005. This document is in not in compliance with Rule 2.03.9. <br />1) Please include a rider to the policy that complies with Rule 2.03.9(3) that states that the insurer <br />will notify the Division of substantive changes in the policy, including termination or failure to <br />renew.
