REPORTING REQUIItEMENTS (cont.) <br />3) A summary of the annual inspection reports, including date, fmdings, and action taken, or [he triennial <br />certification by a Professional Engineer (based on an inspection) that the facility is in compliance with the <br />permit. <br />4) Certification language and signamre by the permittee. (See Part LE.2 of the permit.) <br />The report will be due to the Division on or before February 15, 1999 and cover the period of time between the <br />coverage by this general permit and December 31, 1998. The Division reserves the right to require additional <br />information in the report, and more frequent reports, on a case-by-case basis, as needed. <br />c. Annual Report for Remediation Operations: The permittee will be required to submit an Annual Report, <br />covering January 1 through December 31 of each yeaz, on the overall compliance with the SWMP. The Annual <br />Report shall contain, at a minimum: <br />1) Name of permittee, address, phone number, and permit certification number. <br />2) A report on the facility's overall compliance with the SWMP, including a progress report on all <br />remediation activities. <br />3) A summary of each comprehensive stotmwater facility inspection made, including date, fmdings, and <br />action taken. <br />4) Results and interpretation of any stormwater monitoring performed. <br />5) Certification language and signamre by the permittee. (See Part 1.E.2 of the permit.) <br />The report will be due to the Division on or before February 15, of each yeaz. The enact due date for the <br />permittee's first Annual Report will be listed in their permit certification. The Division reserves the right to <br />require additional information in the report, and more frequent reports, on a case-by-case basis, as needed. <br />d. Si¢natorv Requirements <br />a. Ail reports required for submittal shall be signed and certified for accuracy by the permittee in accordance with <br />the following criteria: <br />1) In the case of corporations, by a principal executive officer of at least the level of vice-president or his or <br />her duly authorized representaive, if such representative is responsible for the overall operation of the <br />facility from which the dischazge described in the form originates; <br />2) In the case of a partnership, by a general partner; <br />3) In the case of a sole proprietorship, by the proprietor; <br />4) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking <br />elected official, or other duly authorized employee. <br />Changes to authorization. If an authorization under pazagraph a. of this section is no longer accurate because <br />a different individual or position has responsibility for the overall operation of the facility, a new authorization <br />satisfying the requirements of paragraph a. of this section must be submitted to the Division, prior to or <br />together with any reports, information, or applications to be signed by an authorized representative. <br />c. Certification. Any person signing a document under pazagraph a. of this section shall make the following <br />certification: <br />
