MEMO <br />DATE: June 27,1991 <br />TO: Bob Nondorf, Division of Purchasing r_ <br />FROM: Tom Gillis, Mined Land Reclamation Division ~~ <br />RE: Hazardous Materials Sampling Project P.O. 1 <br />Enclosed are the certificates of insurance from the low-bid vendor <br />for the above referenced project. Also enclosed is an invoice from <br />the vendor for the required insurance coverage. None of the vendors <br />who bid on the job were informed prior to bid submittal that insurance <br />would be required as part of the job. The low-bid vendor is a one-man <br />consulting firm and does not regulary carry general public liability <br />insurance or workmen's comp. in the amounts which are required by the <br />State. As such, it would appear reasonable and prudent to add the <br />cost of the required insurance coverage to the original bid amount of <br />$2,795.00. The increase of $500.00 to the original bid amount will <br />still result in a savings to the State of $1,332.90 when compared to <br />the second lowest bid amount of $4,627.90. <br />For these reasons, I request that the purchase order amount for the <br />sampling project be increased to $3,295.00 in order to cover the <br />additional cost for insuring the vendor while performing the project <br />work. <br />
