On December 17, 2001, you sent me a letter informing the Division that Holnam, Inc. has changed its name'to
Holcim (US) Inc. The name change requires that certain actions be completed to keep your permit 'in
compliance.
]. You must complete and submit a "Transfer of Mineral Reclamation Permit and Succession of
Operators Application Package" accompanied by a $125.00 fee to transfer the permit from
Holnam, Inc. to Holcim (US) Inc. (application enclosed)
2. You must change the permit financial warranty to show Holcim (US) Inc. as the operator. (the
two necessary bond forms are enclosed) Your present required financial warranty is $268,550.
2. You must complete a replacement performance warranty form changing the name of the
operator to Holcim (US),Inc.(form enclosed)
