Please have your staff send the <br />necessary information or contact the appropriate permittees. <br />Additionally, during a recent joint inspection, the Office of Surface Mining Reclamation and <br />Enforcement (OSM) inspector noted that the operator had not submitted an updated <br />certificate of liability insurance to AFO. A survey of the Colorado MRPs within the AFO <br />library revealed that most MRPs still contain the original certificates, which are now long <br />outdated. A review of the computer tracking of material submitted in 1994 revealed that <br />no Colorado permittees sent AFO a 1994 insurance certificate of liability insurance. <br />Colorado regulation 2.03.9 mandates that each permittee submit a certificate of liability <br />insurance "for inclusion in the permit application," which is the MRP, and that the policy be <br />maintained for the "life of the permit and any renewal thereof." Therefore, each year's <br />certificate revises the MRP and should be sent to AFO to update our copies.
