32. Section 2.05.3(3) of the permit indicates that there aze four planned SAE's. Please <br />provide the design information for all planned SAES. As part of the design information, <br />please include sedimentology demonstrations to show compliance with 4.05.2. <br />33. As indicated in Item 12 above, additional design information will be required for the <br />"traveling" ponds. Additionally, as-built certifications in accordance with 4.05.9(14) will <br />be required for each "traveling" pond. The as-built certification will serve to verify that <br />the ponds were constructed as designed and aze capable of retaining the total runoff <br />volume from the 25-yr, 24-hr precipitation event, <br />34. There aze two "diversion structures" (002and 003) that appear to be impoundments <br />dammed by the perimeter road. These are identified on Attachments 2.05.3(3) 1-1 and 1- <br />2 as diversion structure embankments but there are no designs for these impoundments. <br />Please provide designs for these impoundments in accordance with Rule 4.05.9. <br />35. In the legend of Map 2.04.7-1A there is a discussion of the ground water and surface <br />water monitoring sites that are active versus those that are inactive or discontinued. To <br />make the map more readable it would be helpful to highlight the sites that are included in <br />the cun•ent approved/active-monitoring program, <br />36. Attachment 2.04.7-2 provides ground water monitoring hole completions for wells GW- <br />N41 to GW-N46. Please update Attachment 2.04.7-2, to include a well completion <br />summary for all of the active monitoring wells in the currently approved monitoring plan. <br />At a minimum please provide the following information for all active monitoring wells: <br />formation or seam completed in, surface elevation, top of casing or measuring point <br />elevation, type of casing, diameter of casing, total depth of well, and perforated interval. <br />37. In accordance with Rule 4.07.3(3) please provide abandonment reports for all ground <br />water wells that have been reclaimed or mined through. <br />38. It would be helpful to have the baseline ground water monitoring data located in one <br />attachment for all wells that aze currently active or that may be reactivated in the future. <br />Please update Attachment 2.04.7-3 to include baseline data for all active ground water <br />monitoring wells in the currently approved monitoring plan. Please also consider <br />transferring the baseline data for all monitoring wells into Attachment 2.04.7-3 for ease <br />of reference. <br />SECTION V -PERMIT VARIANCES AND SPECIFIC APPROVALS <br />There are no specific variances and approvals by the Division for New Horizon Mine. <br />New Horizon Mine -10- November 23, 2005 <br />
