Please read the terms and <br />conditions of this permit carefully. To accept this permit, <br />please complete the following:• <br />a. Sign and date both copies of the permit. You should <br />include your printed name and title. Your signature(s) will <br />indicate acceptance of all permit conditions. <br />b. Enclose a check or money order in the amount .of $100.00 <br />made payable to FAO-USAED Sacramento. <br />c. Return both copies of the signed permit to me. <br />After we receive all of the above items, we will sign and <br />return one copy. of the fully executed permit to you. <br />We are also enclosing a Notification of Administrative <br />Appeal Options and Process and Request for Appeal.
