(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1977-"147 <br />INSPECTION DATE: ns/13/07 INSPECTOR'S INITIALS: AP <br />OBSERVATIONS <br />1. This inspection was conducted as part of the Division of Minerals and Geology's monitoring program. Present at the <br />inspection were Kate Pickford, DMG, and Jim Ellison, Gordon Benton, and Roy Stienmier, Holcim. Weather at the time of <br />inspection was clear, windy, and approximately 45 degrees. <br />2. A permit sign was posted at the entrance to the site. Survey corners serve as boundary markers for the site. <br />3. Much of the site has been reclaimed. There is a high wall along the north portion of the disturbed area, but, if stable, falls <br />within the parameters of the reclamation plan submitted and approved under Amendment AM-001 for the permit. <br />Another high wall in the form of a road cut exists to the southwest of the disturbed area. This road cut allows access to the <br />west of the site, and according to the operator, the land owner desires that the road remain. An area to the south still <br />requires grading and reclamation. Currently, this area consists of piles of overburden that will be graded and reclaimed in <br />a manner similar to the rest of the site. <br />4. Much of the reclaimed area consists of bare sandstone. Little or no topsoil occurred at the site prior to mining. The barren <br />surface is broken up with berms which help reduce runoff and supply a location to establish trees and shrubs. Trees have <br />been planted and appear to be relatively successful along these berms. Additionally, trees have been planted in boreholes <br />in the bare sandstone floor in various locations, and appear to be successful. <br />5. Other activity currently occurring at the site includes the installation of a utility tower. <br />6. All previous activities and completed work at the time of inspection appeared to be following the guidelines set forth in <br />the mining plan for the site, and all conditions at the time of the inspection fell within regulations. <br />I & E Contact Address <br />NAME: lames R.
