No. COR-040204, Pitkin County <br />Deaz Sir or Madam: <br />The above facility is permitted under the Metal Mining Operations and Mine-Waste Remediation <br />stotmwater general permit. This permit is about to expire. In order to maintain coverage, you must <br />reapply. We sent you a reapplication form in May, but have not heazd back from you. <br />Any discharge of stormwater from your site, without permit coverage, would be a violation of the Clean <br />Water Act. <br />On the attached Reapplication and Record Verification form you will find the latest information that we <br />have in our records for your facility. Please review the information carefully, make whatever changes are <br />necessary, sign and return it to our office by July 31, 2006. In addition, if available, please provide the <br />email addresses requested in items A and B on the form.
