Please check one below and return this <br />form with your payment to DMG. If a CD is requested and [he permit is too large for a CD, you will be rolled. <br />Please allow up ro 5 business days from the receipt of this signed document for your CD or DVp tq be available. <br />CD d 55,00 per disk ` DYD Q $20.00 per disk <br />Please check if you want a copy of the t'ZrtickView installation institutions. <br />Computer requlmmanta to use the DMG document Imaging CD (or DVD): <br />i. A program called QuickView will be espied to the CD and must be installed on your computer in order to use a pMG imaging CD the <br />first time. Future use of this or other imaging CD's does not require a new installation of the software. <br />2. The CD or DVD drat you purchase RlH51 be in the D: drive. A DVD drive will read a CD or DVD. <br />3. The CD will contain tiles widt the .rif extension. There needs ro be an image viewing application for .rif files on your computer that <br />will handle single and mold-page doc:mrents. On a Windows XP wmputer, the default viewing programs are called "Windows Picture <br />and Fox Viewer" or "Psint" and they do not work with QuickView. The Imaging Viewer that comes with pre-Windows XP computers <br />does work. DMG has deternrined that a program called "IrfanView", which can be downteaded from lrfenV)ew.wm, dots work with <br />QuickView. After downloading "IrfanView", it needs ro be configured as your default .rif viewer. DMG does not support or endorse this <br />software and the licensing and registration are the user's responsibility. There are many other programs that work as well, such as <br />"Imaging Proview" and "Imaging for Windows". <br />Instructions: <br />L insert the CD into your computer. <br />2. Allow time for the CD to install. <br />3. Eject the CD from the computer. <br />4. Reinsert the CD into the computer. <br />5. You are ready ro vines the Permit Files. <br />NDTE: A program called QuickView Autorun, which tuns the QuickView <br />setup program, has also been copied to this CD. if the Aurorun program <br />dons NOT work, you will need to manually install fife QuickView program. <br />Instructions for manuaNy inswlling QuickView are available upon request <br />with the Cp or ten be viewed at www.minina.sw[e.en.us. <br />The Permit B0e dacumeut images coo be viewed fa the roreptlon aroa of the DMG office or they can be purchased on o CD ar <br />DVD.
