Document Imaging CD/DVD REQUEST FORM & INSTRUCTIONS Request for the DMG document images on the following DMG permit file(s): 1) 2)L~ 1~~(~J~ 3) 4) The DMG document images can be copied to either a CD or a DVD. Please check one below and re[um this form y~ith your payment to DMG. If a CD is requested and the permit is too large for a CD, you will be called. CD @ $5.00 per disk ~~~ _DVD @ $20.00 per disk Please check if you want a copy of the QuickView installation ins[mctions. Computer requirements [o use [he DMG document imaging CD (or DVD): I . A program called QuickView will be copied to the CD and must be installed on your computer in order to use a DMG imaging CD the first time. Future use of this or other imaging CD's does not require a new installation of the software. 2. The CD or DVD that you purchase must be in the D: drive. A DVD drive will read a CD or DVD. 3. The CD will contain files with the .tif extension. There needs to be an image viewing application for .tif files on your computer that will handle single and multi-page documents. On a Windows XP computer, the default viewing programs are called "Windows Picture and Fax Viewer" or "Paint" and they do not work with QuickView. The Imaging Viewer that comes with pre-Windows XP computers does work. DMG has determined that a program called "IrfanView", which can be downloaded from IrfanView.com, does work with QuickView. After downloading "lrfanView", it needs to be configured as your default .tif viewer. DMG does not support or endorse this software and the licensing and registration are the user's responsibility. There are many other programs [hat work as well, such as "Imaging Preview" and "Imaging for Windows". instructions: 1. Insert the CD into your computer. 2. Allow time for the CD to install. 3. Eject the CD from the computer 4. Reinsert the CD into the computer 5. You are ready [o view [he Permit Files NOTE: A program called QuickView Automn, which mns the QuickView setup program, has also been copied to [his CD. If the Autorun program does NOT work, you will need to manually install the QuickView program. [nstmctions for manually installing QuickView are available upon request with the CD or can be viewed at www.minines[ate.co.us. The Permit File document images can be viewed in the reception area of [he DMG office or they can be purchased on a CD or DVD. The CD/DVD purchase cost covers the time and labor of making the CD. DMG cannot provide computer related support.
