The Water Quality Control <br />Division will reissue the permit before that expiration date. <br />Our records indicate that you are currently covered by this permit for the above-referenced facility. In <br />order to maintain coverage for this permit past September 30, 2006, you must complete and return the <br />enclosed Reapplication and Record Verification form. This will constitute reapplication for the reissued <br />permit. <br />On the attached Reapplication and Record Verification form you will find the latest nrformation that we <br />have in our records for your facility. Please review the information carefully, make whatever changes are <br />necessary, sign and return it to our office by June 30, 2006. In addition, if available, please provide the <br />email addresses requested in items A and B on the form. An addressed envelope has been included. <br />(Our address is also at the top of the form.) <br />Note that by signing the Reapplication and Record Verification form you are also certifying that an up-to- <br />date Stormwater Management Plan (SWMP) is being implemented at your facility. Therefore, you must <br />ensure that the SWMP in place for your facility is being maintained to accurately reflect potential <br />pollutant sources and management practices at the site.
