After completion of the "Equipment Rental Record" form, the <br />PROJECT MANAGER will obtain the concurrence of the CONTRACTOR as to <br />time and hours each piece of equipment was actually operated in <br />conformance with this specification and furnish a copy of each day's <br />"Equipment Rental Record" form to the CONTRACTOR. <br />1.4 Measurement and Payment <br />The number of hours each piece and type of equipment used as directed by the <br />PROJECT MANAGER under this specification will be counted to the nearest <br />one-half hour. Payment will be made at the unit price established by the <br />Invitation and Bid Sheet and in the purchase order contract for supplemental <br />earthwork for each piece and type of equipment. Such payment shall include <br />the cost of all materials, accessories, labor, maintenance, operating <br />supplies, salaries of operators, and any other expenses incidental to the <br />operation of the equipment in the performance of work in accordance with this <br />specification and bid item(s). No extra payment will be made for the use of <br />the same equipment while performing work on other bid items on the Invitation <br />and Bid Sheet, Bid Schedules or Purchase Order Contract. No additions] <br />payment will be made for hauling equipment to or away from the work site <br />except as may be proper under Mobilization/Demobilization. <br />IV. INSPECTION AND"APP ROYAL OF THE WORK <br />All project tasks shall be inspected by the PROJECT MANAGER upon completion. <br />Payment for work items shall not be made until the PROJECT MANAGER has <br />verified that each task has been completed in accordance with the <br />specifications. In addition, certain construction activities and tasks <br />specified in Section III shall be inspected by the PROJECT MANAGER at specific <br />points within the construction sequence as described below. Subsequent <br />activities or tasks shall not occur until the PROJECT MANAGER has inspected <br />and verified that the task or activity has been completed in accordance with <br />the appropriate specifications. These tasks are as follows: <br />Mine Seal Construction - The PROJECT MANAGER shall be notified by the <br />CONTRACTOR immediately upon completion of back fill work behind the <br />two block wall seals and after construction of the structural steel <br />backstop at the ventilation entry, but before construction of the two <br />concrete block walls begins. Notification shall also be given after <br />completion of masonry work on the two block wall seals but before <br />backfill work in front of the block walls begins. <br />2. Placement of Demolished Structures-at the Base of Fill Areas - The <br />PROJECT MANAGER sha p be notifies by the CONTRACTOR immediately upon <br />completion of placing demolished structures and/or other mining <br />related material along the base of areas to be backfilled and before <br />any fill material is placed over the demolished material. Placement <br />of any earth fill material in these areas shall not occur until the <br />PROJECT MANAGER has inspected the placement operation and verified to <br />the CONTRACTOR that placement work has been done in accordance with <br />the specifications. <br />/ern <br />8%o7F <br />-30- <br />
