.r

Simmons requested Denver East Machinery Company to consider pump
replacement and that issue has been resolved. The old well pump was replaced with a new
pump. The old pump was removed and sent for repairs. The repaired pump will be used
as a stand-by pump for Hazel "A".
The single 2" PVC line from Hazel "A" to the pond has been secured and there are
no leaks. The water level in Hazel "A" has not been recorded as less than 12" of
freeboard.
The goundwater between adit entry and water bulkhead, about 50 feet from the
adit collar, is pumped back to the area behind bulkhead and then up to the pond. There is
no dischazge from Hazel "A".
The pond water evaporation system has been in effect for approximately 10 days.
With a closed loop water system and applying the aeration evaporation system a water
balance is achievable even during periods of inclimate weather as recorded in the on site
Log Book. Without the use of water and unable to use Mill Plant Storage in steady head
tanks, ITEC has water balance. The Land application has not been repaired and will be
the next assignment for the operator with respect to water compliance issues.
On April 14, 1998 the following is noted.
Hazel "A" 22"
Pond 19"
Sprinklers Functional
Pumps Hazel "A" Goulds 4" -new
Adit Inby Sump - OK
Adit Standby Sump - OK
10 HP Pond Pump 34 PSI @ Est. 250 GPM
] 0 HP Standby Pump is in for repairs
The closed circuit is complete.
