As you know, this <unendment is <br />required to incorporate into the reclamation permit for the Boettcher Quarry areas of the plant where <br />cement kiln dust is generated, stored, or transported. Although the Division of Minerals and Geology <br />has not yet completed a full technical adequacy review of the application, it does appear based on a <br />preliminary review that the content of the application is in line with previous discussions between <br />HOLNAM and the Division. <br />Unfortunately, the format of the application HOLNAM submitted must be modified to meet regulatory <br />requirements for a permit amendment. Enclosed with this letter are two application forms that include <br />amendment application filing instructions.. Some of the critical components of the application format <br />are: <br />• Filing an original (ivory colored) signed application form and four copies. <br />• Five copies of all amendment application exhibits (please note that [he regulations require that the <br />person that prepazed the maps sign the map exhibits). <br />• Filing a full copy of [he amendment application and exhibits at the Larimer County clerk and <br />Recorder. Obtain a receipt for the filing and provide it to the Division of Minerals and Geology with <br />your application submittal. <br />• An affidavit stating that a notice of the amendment application is posted at the quarry, <br />The amendment application fee for a quarry operation. <br />Proof that the local Soil Conservation District and County Commissioners have been notified of the <br />amendment application. <br />• Please note that the amendment application requires a full set of application exhibits that would be <br />required for a new permit application except that exhibits that duplicate information in the existing <br />permit aze not required. Since most of the required exhibits would duplicate the existinl; permit in this <br />case, please simply provide a statement for each exhibit that is not being modified [hat there is no <br />change to the permit.
