This is in response to your letter dated September 2, 1997, informing
us that the Hazel A adit had been closed with a cinder block and
concrete bulkhead. The Division conducted an inspection of the Hazel
A on September 5, 1997 (copy enclosed) and found that the operator was
attempting to repair a leak in the wood bulkhead by constructing the
cinder block wall. The wall was partially complete to an approximate
height of 3.5 feet. The operator agreed at that time to stop
construction until a technical revision could be filed and approved for
the structure. Please find enclosed a copy of the operator's design
for the wall submitted in Technical Revision TR-002. You should be
receiving a copy of the Division's adequacy review for the design under
separate cover. The Division will consider any comments you may have
regarding the revision.
