(2) distance from center- line cf highway to pnga~rty tine, (1J numhnr of driveways requested. (4) width of proposed driveway(s) and angle of approach, (5) distance from driveway to road intersection, if any, (G) size and shape of area separating driveways if more than one=- approach, and (7) setback rlisfance of building(s) and/or gasoline pump island(s). ~ - 7'he applicant binds and obligates himself to construct and maintain the driveway approach(es) in accordance with the provinir+ns, aster+fications, end conditicns enumerated in this document. - GENERAL PROVISIONS FIRST: The Applicant represents all partlea in interest. and affirms that the driveway approach(es) is to be constructed by him 1'or the bona fide purpose of securing access to his Property and not for the purpose of doing business or servicing vehicles on the highway right-of-way. ' SECCiND: The applicant shall furnish all lal+or and materials, perform all work, and pay all coats in connection with the amstniction of the driveway(s) and its appurtenances on the right-of-way. All work shall be completed within 30 days of the Permit date. THIB.D: The type of construction shall he es designated and/or approved by the Division of Highways and all materials used shall he of satisfactory duality and subject. to inslmctinn and approval by the Diaision. FIFTH: The Applicant shall assume responsibility for the removal or clearance of snow, ice or sleet upon any portion of the driveway approach(es) even though deposited on the driveway(s) in the course of the Divisieri s snow removal opeta',ions. SIXTH: In the event it becomes necessary to remove any right-of-wag fence, the posts on either side of the entrance shall be s^curely braced before the fence is cut to prevent any slacking of the remaining fence. and all posh and wire removed shag bo turned over to th^_ District representative o[ the Division of Highways. SEVENTH: Nn revisions nr additions shall ho made to the driveway(s) or ih appurtenances on the right-of-way without the wriHcn permission of the Division of Highways. NINTH: Pro•risions and specifications outlined heroin shall apply on all highways under the jurisdiction of the Division of Highways, except that on high:vay connecting links within the corporate limits of municipalities operating under home rule po^:ors the Division regulations shall be subject to conntrrence by the local governing body.
