Both of these forms reference the Canadian Strip Mine, CDMG <br />C-81-026. The Canadian Mine permit shows no association with Tremont Corp. or Titanium <br />Metals Corp. The Division questions whether these insurance certifications refer to a <br />minerals mine? <br />The Division received a Certificate of Insurance for the Canadian Mine, on July 5, 1996. <br />This form reflects K N Energy Inc. as the insured, drawn on National Union Fire Ins. Co., <br />American and Foreign Insurance Co., and Safeguard Insurance Co.. Respective policy Nos. <br />are BE3089971, AST259305, and RST1993159507. This Certificate of insurance complies <br />with Rule 2.03.9 with the exception of the cancellation policy. Rule 2.03.9(3) requires that <br />The policy include a rider requiring the insurer to notify the Division whenever <br />substantive changes are made in the policy, including any termination or failure to <br />renew. <br />Please make the appropriate changes to the policy and provide the Division proof of this <br />change. <br />If you have any questions, please contact me. <br />Sincerely, <br />~- ~,-/' <br />ane .
