Lang <br />Division Director <br />Enclosed is a brief description, maps and bid schedule for the Hawk's Nest Mine Hazardous <br />Waste Removal Project. The reclamation bond held by the State has been revoked and the <br />State is conducting reclamation at the site. <br />Site Showing _ <br />There will be an optional pre-bid meeting held at the Hawk's Nest Mine to show the <br />location of the four (4) barrels of potentially hazardous material on Wednesday, August 30, <br />1995 at 10:00 a.m. Please see attached map for meeting location. <br />Bid Submission and Evaluations <br />Bids shall be submitted by 4:00 p.m. on Tuesday, September 5, 1995 by telephone or fax to <br />Erica Crosby or Maggie Van Cleef. Please leave a voice mail message for either Erica or <br />Maggie if neither can be reached. The attached bid schedule must be submitted by the <br />successful bidder. Award will be made for lowest grand,total bid for each item 1 and item <br />2. The number of barrels treated for burner blender or incinerator stated on the bid <br />schedule is for the purposes of comparing bids only. The actual number of barrels treated <br />for burner blender or incinerator will be determined by the results of bid item 1. State <br />reserves the right to award separate purchase order contracts for item 1 and item 2 if it is <br />in the best interest of the State. <br />Bid Award <br />The apparent low bidder must submit an itemized bid on company letterhead and a <br />certificate of insurance with the following limits: <br />1. Standard Worker's Compensation and Employer Liability, as required by State <br />Statute including occupational disease, covering all employees at the work site. <br />
